Definition

Medical transcription is the process of producing reports from dictations provided by medical providers of the details of patient office visits, clinical, operative procedures, and other encounters in the medical arena. By law, every interaction of a patient in any medical institution/provider has to be documented in some form or way.

These documentations are formalized into patient's medical records through dictation and subsequent transcription. In addition to typing the report into the desired format, it is also important to verify the dictation for both medical and English-language accuracy so that the final report presents a clear, medically accurate representation of the encounter between patient and provider.
